Partial or generalized tonoclonic attacks as monotherapy and in combination with other anti-convulsion remedies; additional therapy in curing of Lennox-Gastaut syndrome fits (in adults and in children); first diagnosed epilepsy (in adults and in children of more than 2 years of age).

Topiramate is anti-epileptic medical drug.

5. Posology and Administration

Topiramate should be taken orally (regardless of meal). In adults when there is monotherapy: the starting dose is 25 mg once daily (every night at bedtime) during one week, then the dose should be increased by 25–50 mg/a day (it will be divided in 2 intakes) with an interval of 1–2 weeks up to the attainment of a clinical effect; a recommended dose is 100 mg/a day, maximal daily one is  500 mg (for some patients with refractory forms of epilepsy a dose should be up to 1000 mg/a day). For children of more than 2 years of age: in the first week of the treatment there will be administered 0,5–1 mg/kg every night at bedtime, then a dose should be increased by 0,5–1 mg/kg/a day (divided into 2 intakes) with an interval of 1–2 weeks; recommended doses are 3–6 mg/kg/daily (for children with recently diagnosed partial fits it should be up to 500 mg/a day).
In combination with other anti-convulsion drugs: adults — 25–50 мг mg every night at bedtime during 1 week, then a dose should be increased up to 25–50 mg with an interval of 1–2 weeks; and the drug should be taken in 2 intakes; the minimal efficient dose is 200 mg/a day, an average daily dose should be 200–400 mg, the maximal one is 1600 mg. In children of more than 2 years of age: — 1–3 mg/kg/a day every night at bedtime during one week, then a dose should be increased by 1–3 mg/kg with an interval of 1–2 weeks and should be taken in 2 intakes; a recommended daily dose is 5–9 mg/kg.

6. Counterindications

Counterindications to Topiramate administration may be intolerance of any of the drug’s component, pregnancy, breastfeeding, childhood (safety and efficiency of the drug in children under 2 years of age are not determined).

On the part of nervous system and sense organs: increased fatigability, balance problem in standing, thought disorder and attention concentration, emotional instability, consciousness fog, dizziness, feeling of skin numbness, reduction of a superficial cutaneous touch, drowsiness; seldom – agitation, amnesia, speech disorder, depression, diplopia and other vision impairments, conjunctivitis, high speed unintentional oscillatory movements of eyes (up to several hundreds a minute), perversion of taste.
On the part of organs of digestive tract: loss of appetite, gingivitis, nausea.
Others: decrease of the body mass, pain in the stomach, cold fit, decrease of the number of leucocytes in blood, panting, edemas, nose bleeding, formation of stones in renal pelvis or in renal tubs, presence of erythrocytes in urine, itching in groin area, loss of sense of hearing, painful menstruations, libido weakening, impotency.

11. Pregnancy and Lactation

Topiramate is prohibited for using during pregnancy, likewise in the period of breastfeeding.

Topiramate should be administered to patients who are engaged in potentially hazardous activities which require alertness and swiftness of psychomotor actions with precaution, as the drug may provoke drowsiness and headache.
