3. Indications for Use.

For systemic use: infectious-inflammatory pernicious diseases provoked by the microorganisms sensitive to the drug: pneumonia, abscess of lung, empyema, osteomyelitis, endometritis, adnexitis, purulent infections of skin, soft tissues, wounds, peritonitis. Prevention of peritonitis and intraabdominal abscesses after perforation or trauma of bowels (in combination with aminoglycosides). It acts as a reserve antibiotic in the infections provoked by staphylococcus stocks and other gram-positive microorganisms resistant to penicillin.
For local use: vaginoses provoked by the microorganisms sensitive to the drug.
For outward application: acne vulgaris.

4. Action

In therapeutic doses Clindamycin has an effect on a microbe cell, breaking an intracellular synthesis of albumen. The drug is highly active as for gram-positive, likewise as for some gram-negative microorganisms.

5. Posology and Administration

Posology and administration are individual. Adults are to be administered orally 150-450 mg 4 times daily. In infectious-inflammatory diseases of abdominal cavity and pelvic organs the administration is intramuscular and it is daily from 1.2 to 2.7 g in 3-4 introductions. Maximal daily dose of intramuscular introduction is 4.8 g., it is not recommended to introduce more than 600 mg in one intramuscular injection. If the disease is pernicious the drug is to be introduced with intravenous drip-feed in a dose of 600-900 mg (maximal single dose of intravenous introduction is 1.2 g) at the velocity of not more than 30 mg/min., the length of a treatment course is 10-14 days.
In children orally: more than 1 month of age – the drug is administered in a daily dose of 8-25 mg/kg in 3-4 intakes; in children with the body mass under 10 kg 37.5 mg (1/2 tea spoon) 3 times daily. Newborns are parenterally introduced 15-20 mg/kg of the body mass daily, babies older 1 month are administered 20-40 mg/kg daily, and repetition factor is 3-4 times.
Vaginal cream is administered 1 dose (1 applicator) before sleep; the treatment course is 7 days.
Gel of outward application is put on an affected region 2-3 times/daily.

6. Counterindications

Evidenced renal dysfunction and compromised liver function, pregnancy, lactation, hypersensitivity to Clindamycin.

7. Should Be Taken with Precautions, if…

The drug is to be administered with caution in cases of myasthenia, bronchial asthma, ulcer colitis (in anamnesis), severe kidney and/or liver failure, pregnancy, lactation period. With caution it should be applied in elderly patients.

Symptoms: enhancement of the evidence of side effects.

10. Side Effects

Impairment of nervomuscular capacity, impairment of the system of blood formation and blood.
Impairment of arterial tension (high, low) when intravenous introduction is quick, cardiac and respiratory standstill.
On the part of intestinal tract organs: pain in stomach, nausea, vomiting, diarrhea, dysbacteriosis, yellow sickness, liver dysfunctions, elevation of appetite, when big doses are introduced intravenously – unpleasant metallic after-taste in the mouth.
Allergic reactions: rash of mild and medium evidence, hives, itching;
Moreover: superinfections; reactions in the place of introduction – soreness, induration, abscess (when intramuscular introduction), thrombophlebitis (when intravenous introduction).

11. Pregnancy and Lactation

During pregnancy Clindamycin is counterindicated.
In lactation period Clindamycin is to be administered only on strict medical conditions. For the use during lactation it is necessary to decide upon the cancellation of breastfeeding.
