3. Indications for Use.

-acute venous thrombosis and abrupt blocking of the bloodstream with a detached piece of thrombus of pulmonary artery (together with heparin),

-as a supplementary measure in surgical or medicated (thrombolytic) care delivery of thrombosis, likewise when there is electrical conversion of atrial fibrillation,

-repeated pulmonary embolism,
-presence of valve prostheses or prostheses of blood vessels (combination with acetylsalicylic acid is possible),
-thrombosis of peripheral, coronary and cerebral arteries,
-secondary prevention of thrombosis and thromboembolia after myocardial infarction and when there is atrial fibrillation.

4. Action

The drug suppresses synthesis of vitamin К-dependent factors of blood coagulation. Concentration of these components in blood decreases, whereas the process of blood coagulation is retarded.

5. Phonology and Administration

The drug is administered to surgery patients, the drug is recommended to be taken 2-3 days before surgery.
The starting dose of the drug is 2.5-5 mg/daily.
The further dosage regimen is to be set up individually. The maximal dose may exceed the minimal one not more than 4 times (this factor is strongly individual).
For elderly people and weakened patients the drug is administered in lower doses.
Full daily dose should be taken in one intake and in the same time of the day.
The length of the therapy course depends on the clinical state of the patient. The treatment may be withdrawn at once.

6. Counter indications

Hypersensitivity, acute bleeding, severe diseases of liver or kidneys, bad arterial hypertension, acute DIC-syndrome, protein C and S malnutrition, bleeding diathesis, thrombocytopenia, gastric and duodenal ulcer in aggravation stage, encephalorrhagia, alcoholism, kidney failure, pregnancy.

7. Should Be Taken with Precautions, if…

With caution the drug should be administered when the process of blood coagulation is impaired, when there are thrombocytopenia, high arterial tension, gastric and duodenal ulcer in aggravation stage, in compromised functions of liver and kidney, encephalorrhagia and it should be administered with caution to chronic alcoholism patients.

8. Interface with Other Medications

A series of medications interplays with Warfarin, thus, the patients should be notified on the necessity of consultations with the attending doctor before the start of administration of other medications or before the change of dosage regimen of the already administered medications.

9. Overdosing

Patients should be strongly advised of the necessity of following of the recommended regime of dosage of the drug.
Symptoms: bleeding sickness, effusions of blood.

10. Side Effects

On the part of blood coagulating system: often – bleeding sickness.
On the part of intestinal system: seldom – diarrhea, elevation of liver ferments activity.
Dermatologic reactions: seldom – eczema, skin necrosis, loss of hair.

11. Pregnancy and Lactation

The drug should not be administered to pregnant women as it may be progression of bleedings in the fetus and its death. In case if pregnancy has emerged the drug should be withdrawn immediately. Warfarin may be used during breast-feeding but for full safety it is recommended to refrain from breast-feeding during the first three days of Warfarin therapy.
