- Indications for Use.
Depression therapy of various forms and structures in adults. The drug treats panic disorders, obsessive fears and various phobias. The treatment of chronic dissociations not strongly manifested in a patient’s behavior.
- Action
Antidepressive performance usually develops after 2-4 weeks of treatment. Citalopram does not affect on cardiac conduction system and arterial tension, hematologic factors, liver and kidney functions, it does not provoke weight increment.
- Posology and Administration
Citalopram is administered once daily. The drug may be used in any time of the day regardless meal.
Depressions: depressions therapy is to be started with an intake of 20 mg daily depending on an individual reaction of patient and the depression severity; a dose may be increased up to the maximal one – 60 mg daily.
Panic disorder: during the first week of treatment a dose of 10 mg daily is recommended, and then it is to be increased up to 20 mg daily. A 24-hours dose in relation to the individual reaction of a patient may be increased. Maximal dose is 60 mg daily.
Chronical disorder against various phobias: an initial dose is 20 mg daily. If needed a dose may be increased. The maximal dose is 60 mg daily.
Elderly patients (over 65 years of age). A recommended daily dose for elderly people is 20 mg. In relation to the individual therapy and the severity of depression a dose may be increased up to the maximal one - 40 mg daily.
In liver failure the drug administration should be limited by minimal recommended doses. The maximal dose is 30 mg daily.
No dose correction is needed in small and moderately manifested kidney failure. In order to decrease the probability of the emergence of psychic tension it is recommended to use low starting doses.
- Counterindications
Hypersensitivity to Citalopram.
Pregnancy, breastfeeding, childhood (it has not been determined safety and efficiency of use for children). Drug dependence. Liver and/or kidney failure, convulsion attacks. Age under 15, liver cirrhosis, bleeding.
In case of epilepsy the drug administration should be cancelled.
- Should Be Taken with Precautions, if…
In diabetes, also in patients over 65 years of age and in the progression of manic condition it should be used with caution under the care of a physician.
- Interface with Other Medications
It is not recommended to take this drug simultaneously with other medications without doctor’s orders.
Severe unwanted reactions in a simultaneous intake are possible.
- Overdosing
Symptoms: dizziness, dearticulation, coma; occasionally – grand mal epilepsy, high perspiration, tachycardia, oxygen lack, hyperventilation of the lungs, nausea, vomiting.
- Side Effects
Nausea, anorexia, constipations; seldom - taste disturbances; possibly – vomiting, dry mouth, change of laboratory showings of liver functions. Insomnia or drowsiness, dizziness, weakness; possible – vision disorders, convulsion attacks, tremor, disorders of motor function, hallucinations, mania, mental confusion, strong emotional excitement, psychic tension, depersonalization, panic attacks, high irritability, dizziness, headaches and nausea. Decrease of libido, impotency, and retention of urination. The frankness of these reactions is not significant, and their duration is limited.
A hypertonic crisis is possible. Side effects most often may be developed on the 1-st or the 2-nd week of the therapy and after that they usually become less intensive and arise less frequently in the therapy continuation. In progression of manic condition the drug administration should be stopped.
- Pregnancy and Lactation
The drug should not be administered to pregnant and breastfeeding women if a potential clinical advantage does not exceed a theoretical risk, as the safety of the drug in the period of pregnancy and lactation is not determined.
- Driving
Taking this into account while administering Citalopram for depression, a patient should be warned about a possible decrease of the ability of attention concentrating and the speed of psychomotor reaction in vehicle driving and the work with mechanisms.
