3. Indications for Use.

    — acute diarrhea (symptomatic therapy);
    — chronic diarrhea;
    — ileostomy (symptomatic therapy for the reduction of quantity and volume of fecal matter, likewise for making its consistency more solid).

5. Posology and Administration

 When there is acute diarrhea the starting dose is 2 capsule for adults and 1 capsule for children of over 5 years of age; later it will be administered 1 capsule after each defecation act, in case of liquid stool.
When there is chronic diarrhea the starting dose for adults should be 2 capsules daily, for children – 1 capsule daily. A daily dose should be established individually in such a manner that it should reduce the number of defecation acts to 1-2 a day; therefore it will be from 1 to 6 capsules.
The maximal daily dose for adults is 8 capsules, for children the intake should be based on 3 capsules per 20 kg of the body mass. Children from 2 to 5 years of age should be administered a solution for oral usage in a dose of 1 graduate cup (about 5 ml) per each 10 kg of the body mass 2-3 times daily. Upon emerging of a normal stool and in the absence of defecation acts during 12 hours Loperamide therapy should be withdrawn.

6. Counterindications

   – acute dysentery and other infections of digestive tract (also provoked with Salmonella spp., Shigella spp., Campylobacter spp.);
   – bowel obstruction (also as appropriate vermicular movement suppression should be avoided);
   – diverticulosis;
   – acute ulcer colitis;
   – pseudomembranous enterocolitis (diarrhea provoked by antibiotics);
   – I-st trimester of pregnancy;
   – lactation period;
   – childhood under 6 years of age;
   – hypersensitivity to Loperamide and/or other components of the drug.

7. Should Be Taken with Precautions, if…

Care should be taken in administration of Loperamide in cases of liver failure. The drug intake should be stopped immediately if constipation or abdominal distension is noticed, especially in AIDS patients. Patients with compromised liver function should be under a careful supervision aimed to detect timely the signs of a toxic central nervous system affliction. In the period of treatment dieting and liquid volume replacement are recommended.

8. Interface with Other Medications

9. Overdosing

Symptoms: central nervous system suppression – stupefaction, loss of coordination, drowsiness, considerable dilatation of a pupil, increased muscle tonus, respiratory depression, bowel obstruction. Children in a greater degree are sensitive to actions on CNS.

10. Side Effects

Dry mouth, nausea, vomiting, pains and discomfort in the stomach, meteorism, constipation, bowel obstruction; fatigability, drowsiness, dizziness, loss of coordination, apathy; increased skeletal muscles tonus, respiratory depression, allergic reactions (dermatic).

11. Pregnancy and Lactation

Loperamide  is counterindicated in the I-st trimester of pregnancy, in the II-nd and in the III-d trimesters of pregnancy it may be administered only in the cases when supposed favor of its therapy exceeds potential risk for the fetus.
Loperamide in small quantities excretes into breast milk - therefore if there is a need to use the drug in the period of lactation, breastfeeding should be stopped.

In the period of treatment it is necessary to refrain from vehicle driving and potentially hazardous activities which require elevated concentration of attention and high speed of psychomotor ability.
