3. Indications for Use.
Osteodystrophy of kidney genesis, climacteric osteoporosis, post-surgical and idiopathic hypoparathyreosis, pseudohypoparathyreosis, pseudo-deficient vitamin-D-dependent rachitis, native vitamin-D-resistent rachitis (phosphate-diabetes).
4. Action
D-vitamin-form. It favors absorption of calcium in intestinal tract, regulates bone mineralization, and stimulates osteoblasts.
5. Posology and Administration
It is to be taken orally; the optimal day’s dose is matched according to the level of calciemia. A starting day’s dose is 0,25 mkg. If clinical indication does not improve during 2–4 weeks of treatment, the day’s dose will be increased by 0,25 mkg up to an average one (0,5–1 mkg/daily) and higher. In climacteric osteoporosis the dose will be 0,25 mkg 2 times a day. In kidney osteodystrophy a starting daily dose is 0,25 mkg, it is enough 0,25 mkg every other day for patients with a normal level of serum calcium or an imperceptibly decreased one. In hypoparathyreosis and rachitis a starting dose is 0,25 mkg daily (in the morning). In phosphate- diabetes it is administered a supplementary indication of phosphorus-containing drugs. In children it is administered 0,01–0,1 mkg/kg.
6. Counterindications
Hypersensitivity, hypercalciemia, kidney osteodystrophy with hyperphosphatemia, hypervitaminosis D.
7. Should Be Taken with Precautions, if…
It is necessary to define the content of calcium and creatinine in 4 weeks, 3 and 6 months after the start of treatment, then it is necessary to do it with intervals of 6 months. A daily dose in 5 mkg and more should be administered with a due care (clinical trial, determination of the level of calcium, of phosphoros and of alkaline phosphatase activity in blood should be done 2 times a week).
Caution should be taken in cases of atherosclerosis, pulmonary tuberculosis (active form), hyperphosphatemias, phosphate nephrourolithiasis, chronic heart disease, chronic kidney disease, sarcoidosis or other granulomatoses, pregnancy, lactation period, elderly age (may provoke the development of atherosclerosis), childhood age.
8. Interface with Other Medications

9. Overdosing
Symptoms of chronic intoxication: dystrophy (weakness, loss of weight), dysesthesia, eventual fever with thirst, polyuria, water deprivation, apathy, retardation of growth and infections of urinary tracts. Focal calcification of cortical layer of kidneys, of cardiac muscle, lungs and duodenal are after-effects of hypercalciemia.
How to treat an eventual overdosage: immediate lavement of stomach or administration of a vomitive for the prevention of further absorption. If an overdosage is suspected call your doctor immediately.
10. Side Effects
Hypercalciemia, anorexia, headache, vomiting, gastralgia, intestinal colic, calcinosis of soft tissues, increment in activity of liver transaminases, constipations, dystrophy, dysesthesia, fever, thirst, polyuria, water deprivation, dizziness, apathy, retardation of growth, infections of urinary tracts.
11. Pregnancy and Lactation
Rocaltrol is to be administered to pregnant women only in the case if an expected utility of the treatment surpasses a possible risk for the fetus.
Rocaltrol passes into human milk. In consideration of possible hypercalciemia in mother and side effects in infants, the women taking Rocaltrol may continue breastfeeding only in case of a careful medical monitoring of the concentration of calcium in cerum of mother and baby.
