3. Indications for Use.

There are indications for the administration:
  — treatment of depressive states, among them those accompanied with a feeling of anxiety, with or without mania, likewise treatment of social depressions.
    — prevention of initial or chronic episodes of depression (after a leading treatment), likewise treatment of a post-traumatic stress condition.
    — treatment of obsessive-compulsive disorder (obsession).
Also the drug is efficient in the treatment of pre-menstrual syndrome.

The drug acts positively in various forms of depressions, favor normalization of temper and enhancement of positive emotions in post-traumatic disorders, likewise in the period of pre-menstrual syndrome in women.   

5. Posology and Administration

Zoloft is administered once daily in the morning or in the evening. The tablets are to be taken regardless of meal.
In depressions and obsessive-compulsive disorders the treatment should be started from a dose of 50 mg/daily.
The treatment of panic disorders, acute anxiety states and social phobia is to be started from the dose of 25 mg daily, which is to be increased after one week up to 50 mg/daily. The use of the drug according to such a scheme allows reducing the frequency of the early adverse effects of treatment typical for panic disorders.
If the application effect is not sufficient it is possible to increase a daily dose in the patients who take 50 mg/daily. A dosage rate should be increased with an interval not oftener than once a week up to the maximal recommended dose, which is 200 mg daily.
An initial performance may be observed in 7 days after the beginning of the treatment, but a full effect is usually reached in 2-4 weeks (or even during a longer period in OCD).
In a long-term supporting therapy the drug is to be administered in the minimal efficient dose which further on is to be changed in relation to clinical performance.
In children and in teenagers of 13-17 years of age suffering from obsessive-compulsive disorders Zoloft therapy should be started from a dose of 50 mg/daily. In children of 6-12 years of age suffering from obsessive-compulsive disorders the therapy should be started from a dose of 25 mg/daily, after one week it may be increased up to 50 mg/daily. In further days if the drug performance is not sufficient a dosage rate may be increased step by step from 50 mg/daily up to  200 mg/daily, as and when necessary. But in order to avoid overdosage, while increasing a dose by more than 50 mg it is necessary to take into consideration a lesser body mass in children as against that of in adults. A dose should be changed with an interval not less than 1 week.
For elderly age the drug is be taken in the same dosage rate as for younger patients.
In patients with disorders of liver function lesser doses should be used or intervals between the drug intakes should be enlarged.
In patients with disorders of kidneys function there is no need to correct doses with regard to intensity of kidney failure.

6. Counterindications

Hypersensitivity, pregnancy; lactation period, neurological disorders (among them mental retardation), manic states, epilepsy, liver and/or kidney failure, decrease of the body mass, childhood under 6 years of age.

7. Should Be Taken with Precautions, if…

As all the drastic antidepressants the drug is to be taken on doctor’s order only, for the avoidance of possible adverse side effects tell your doctor if you have taken any other drugs recently.
In the treatment period it is necessary to exercise a strict control over the patient’s behavior in depression state (risk of suicidal attempt) until there is no sufficient improvement of the results of the therapy. Patients with disorders of liver function should be administered lesser doses. Women of child-bearing age should use adequate methods of contraception.

9. Overdosing

Overdosing is unlikely (the drug has broad margins of safety). In rare cases the main symptoms: anxiety, drowsiness, heart function failure, dilatation of pupil, nausea, vomiting, tachycardia. There are known lethal cases in overdosage in combination with other drugs or with alcoholic beverages (in such overdosages side effects are significantly enhanced).

10. Side Effects

Allergic reactions, bleedings (also nose bleeding), heartbeat, dry mouth, decrease of appetite. Seldom – increase of appetite (possible result of abolition of depression), nausea, vomiting, unstable stool, diarrhea, spasms in ventricle or in stomach, decrease of the body mass; headache, dizziness, insomnia, drowsiness, jerks of limbs and the head, movement disorder (change of gait), hyperexcitability, convulsions, benumbed sensation, creeping sensation and tingling, depression symptoms, hallucinations, aggressiveness, agitation, state anxiety, mental affection, flushed skin or “flushes”, vision disorders (including blurring of vision), yawning, high perspiration, disorders of sexual function (retention of emission of seminal fluid, decrease of potency and/or libido), increased pain in menstruation (in women), excretion of milk from lacteal glands (in men and women), hormonal disorders, mood swings, mania, habituation syndrome, skin evidences of rash as watery bubbles, skin rash and itching.

11. Pregnancy and Lactation

It is not recommended for the use in pregnancy and breastfeeding.

12. Driving

In the period of therapy it is recommended to take caution in driving of vehicles and in doing the other potentially hazardous activities which require elevated concentration of attention and swiftness of psychomotor actions.
