Tretinoin
- Indications for Use.
Acute promyelocytic leukemia (for remission inducing /temporary easement or cancellation of the disease manifestation/) both in the patients who have received no therapy before and in setbacks (reemergence of the disease signs) or refractoriness (unresponsiveness) to standard therapy (Rubomycinum or Cyrarabine). For local use – vulgar pimples, including those which form comedos (sebaceous plugs at the entrance of hair follicles), papules, pustules (vesicles filled with pus on the skin), confluent pimples. Favre-Rakusho (nodular cystic skin elastosis with comedos).
- Action
Antitumoral, keratolythic, anti-seborrhea, comedolythic anti-inflammatory local performance.
The drug enhances the proliferation of the cells in papillary layer; it decreases adhesion of the cells participating in the formation of vulgar pimples.
When open pimples are treated the drug favors surfacing of the skin without any signs of inflammation. When closed pimples are treated the drug favors its transition into the open pimples or papules, which later heals without their scarring after keratic plug has been removed.
Tretinoin prevents from the formation of new pimples. It takes anti-seborrhea, anti-inflammatory, keratolythic, immunostimulating action.
- Posology and Administration
The drug is used locally in the form of cream or lotion. Fine layer of it should be applied on impaired parts 1—2 times a day. The treatment course lasts 4—6 weeks or more (up to 14). Usually Tretinoin is used in 0,05 % concentration. If the performance is not sufficient and the drug tolerance is good 0,1 % concentration is to be applied. Orally:45 mg/m2 /daily in 2 intakes during 90 days or during 30 days after the achievement of full remission with subsequent consolidating cytostatics chemotherapy. Maximum permissible dose for adults is 195 mg/m2/daily, for children - 60 mg/m2/daily. In kidney and/or liver failure a dosage rate is to be reduced to 25 mg/m2 /daily, in patients with solid tumors it is recommended 1/3 from the maximal dose, in children a dose should be less than maximally recommended.
When Tretinoin is used externally it’s the entrance of into the eyes, mouth and mucous coatings should be avoided (in case of entrance wash the drug out with water right away). During the treatment one should avoid the sunlight. If a patient is sunburnt the treatment can be started after the weakening of the tan. In the first week of the therapy the emergence of pimples may be enhanced - it is due to the drug performance on profoundly laid pimples which were not seen before the treatment begins.
- Counterindications
Hypersensitivity, for local application one should pay attention to acute inflammatory (including eczematous) skin impairments (dermatitis, seborrhea etc.), rosacea, wounds, skin burns, light sensitivity, diseases of the straight intestine.
Pregnancy, lactation period, childhood under 12 years of age (in all indications), under 18 years of age (in the treatment of adverse influence of the sun rays), more than 50 years of age (in the treatment of adverse influence of the sun rays).
- Should Be Taken with Precautions, if…
Seborrhea dermatitis, eczema, sunburns. No reliable contraception in women of childbirth age, intracranial hypertension (especially in children), hypercholesteremia, hypertriglyceridemia, leucocytosis (more than 5·109/l), impairment of liver and kidneys function, pancreatitis, diabetes, chronic intoxication (including that of alcoholic), age more than 50, childhood (for oral administration under 1 year of age, and under 12 years of age in local use).
- Interface with Other Medications
If you are taking some other medications simultaneously with Tretinoin, tell your doctor in order to avoid adverse interaction of the drugs and enhancement of side effects.
- Overdosing
Symptoms: “Vitamin A 1 syndrome”. If you suspect overdosing call your doctor immediately!
- Side Effects
In oral administration of Tretinoin about 25% of patients are reported to have the progression of “Vitamin A 1 syndrome”. If this syndrome is uncured a lethal (death) outcome is possible. Also there may occur xerodermia (dry and exfoliative skin), dry mouth, cheilitis (inflammation of the skin interface and/or mucous of lips), rash, hydrops, nausea, vomiting; high concentration of triglycerides in the plasma, high cholesterol, high activity of transaminases (ferments). Less frequently may happen erythema (limited redness of the skin), itching, high perspiration, cellulite (fibrous panfacial cellulitis), alopecia (partial or full loss of hair), dry mucous coatings, exfoliative dermatitis (redness of the skin of entire body with its evidenced peeling), xerophthalmia (dry eye mucouses); pains in stomach, diarrhea (runs), constipation, bullous stomatitis (inflammation of mucous coating of mouth cavity); heart rhythm disorders; cough, pleural fluid, rhinedema, panting, pharyngitis (inflammation of the throat), rales in the lungs, stridorous (noisy) breath; dizziness, conscious disorders, high intracranial tension, anxiety, depression (low spirits); disorders in vision and hearing; fever, chill, change of weight, pains in muscles and bones, pains in chest and the back, high bleeding, overlay of infection (including pneumonia, ceptical fever /a form of blood contamination with microorganisms), general weakness and drowsiness. Clinical situation will show whether it is necessary to cancel completely the therapy or to suspend it.
In local use redness, burn feeling, peeling of the skin in application places may occur; in rare cases the emergence of edemas, blisters and crusts is possible. Likewise photosensibilisation (high sensibility of body to the sunlight) may occur.
- Pregnancy and Lactation
Pregnancy (the drug takes teratogenic /provoking impairments in the fetus’s development/action). If administration of Tretinoin is needed nursing mothers should stop breastfeeding.
